Insurance, Flexible Spending Accounts and Retirement Savings Benefits

For all the insurance and benefit programs listed below, full-time employees are eligible on the first day of the month following one month of employment with the company.

Medical

Medical insurance benefits from United HealthCare provide comprehensive health care at approved facilities and through network and non-network doctors and health care professionals. A prescription drug program is also included. EPC offers four different coverage levels: Employee Only, Employee & Spouse, Employee & Children or Family coverage. Employees pay a portion of their medical insurance premium on a semi-monthly basis. EPC has established a tiered system of contributions, whereby employees who earn higher salaries pay a higher percentage of their premiums than those who fall into lower salary ranges.

Dental

Dental benefits through United HealthCare provide comprehensive dental care through network and non-network dental professionals. Covered treatments include, but are not limited to, routine exams, dental x-rays, oral surgery, endodontic and children’s orthodontics. EPC offers two different levels of coverage: Employee Only or Family coverage. Employees pay a portion of their dental insurance premium on a semi-monthly basis.

Vision

Voluntary Vision insurance benefits from United HealthCare provide comprehensive vision care through network and non-network vision professionals. Covered benefits include, but are not limited to eye exams, frame allowance, contact lens benefits, lens options and discounts on additional pairs of glasses. EPC offers four different coverage levels: Employee Only, Employee & Spouse, Employee & Children or Family coverage. Employees pay their vision insurance premium on a semi-monthly basis.

Life & Accidental Death & Dismemberment (AD&D)

EPC provides 100% employer-paid life insurance in the amount of 2 times an employee’s eligible earnings plus $10,000 to a maximum of $110,000. Additional coverage is available if the death is a result of an accident or if an employee loses a hand, foot, or sight. Coverage is provided through The Reliance Standard Life Insurance Company.

Dependent Life

EPC provides 100% employer-paid life insurance for employees’ dependents in the amount of $5,000 for spouse and each dependent child.

Long-Term Disability

EPC provides employer-paid group long-term disability insurance through The Reliance Standard Life Insurance Company as a source of income protection. After an elimination period of 90 calendar days from the onset of a qualified disability, this coverage provides up to 60% of eligible monthly earnings up to a maximum monthly benefit of $10,000 per month.

Medical Flexible Spending Account

Employees have the option of contributing up to $2,749.92 annually on a pre-tax basis into this account. Those funds can then be used for reimbursement of eligible medical expenses. The company contributes $25 per month after an employee has completed the waiting period.

Dependent Daycare Flexible Spending Account

Employees have the option of contributing up to $4,999.92 annually on a pre-tax basis into this account. These funds can be used to pay for daycare expenses for children up to the age of 13.

Voluntary Insurance Plans

EPC offers a variety of insurance products that employees may purchase.

CSI 401(k) Plan

The CSI 401(k) Plan uses Fidelity Investments as the Trustee of the plan. Employees can defer up to 60% of their income to the yearly IRS maximums.  The company matches up to 3% of base pay earned for any employee who is an active employee on December 31.  The match is only contributed once per year.  There are more than 31 different fund options that cover a variety of investment styles.

Paid Time Off Benefits

Personal Day

After one (1) year of employment, eligible employees receive two (2) personal days per anniversary year.

Sick Time

Eligible full-time employees will receive two (2) days of paid sick leave immediately following the date of hire, and 5 (five) more days of paid sick leave after completing ninety (90) calendar days of continuous employment.

Vacation Days

Eligible full-time employees will receive 5 (five) days of vacation PTO immediately following the date of hire, and 5 (five) days of paid vacation PTO after completing ninety (90) calendar days of continuous employment. The full-time employee is immediately eligible to take their awarded hours of vacation although these hours will actually continue to be earned during the remainder of the employee’s first year of employment.

Holidays

Specific days off vary from year to year. However, generally EPC observes the following holidays: New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, and the day after; and Christmas Day.

Short-Term Disability

After you have been employed for one year, if you are temporarily unable to work, EPC may continue some of your salary for a limited period depending on your length of continuous employment with the company. Benefits are payable based on seniority with the company up to a maximum of 65 workdays per anniversary year of short-term disability pay after satisfying a five-day qualifying period. Full pay is provided for covered days during an employee’s short-term disability.

Other Types of Time Off

EPC also provides paid time off for Jury Duty and Bereavement Leave for covered relatives. The company provides unpaid time for Military Leave.

Business Casual Dress

EPC desires to have a business-casual dress environment (which we call “Relaxed Dress,”) which means that we want our employees to dress informally and comfortably, but still appropriate for a business setting.

Confidential counseling services are available at no cost to the employee. EAP services are provided by H&H Health Associates.
